Add an Additional Cardholder - Non-Profit Business

Step 1: Download Business Maintenance Form 


Step 2: Fill out the contact section of the form

  • Fill out company name, contact email, phone number, billing address, and billing/control account number


Step 3: Choose the appropriate option

  • If you are looking to add an additional cardholder with their own unique card number select additional cardholder(s)
  • If you are looking to add an additional cardholder onto an existing card number select additional authorized user(s)


Step 4: Fill out the table

  • Make sure you provide DOB and SSN/EIN as these will be used for verification of this cardholder

Step 5: Sign the bottom of the form
