Step 1: Download Business Maintenance Form
Step 2: Fill out the contact section of the form
- Fill out company name, contact email, phone number, billing address, and billing/control account number
Step 3: Choose the appropriate option
- If you are looking to add an additional cardholder with their own unique card number select additional cardholder(s)
- If you are looking to add an additional cardholder onto an existing card number select additional authorized user(s)
Step 4: Fill out the table
- Make sure you provide DOB and SSN/EIN as these will be used for verification of this cardholder
Step 5: Sign the bottom of the form
