Update Cardholder Name - Non-Profit Business

Step 1: Download Business Maintenance Form


Step 2: Fill out the contact section of the form

  • Fill out company name, contact email, phone number, billing address, and billing/control account number

Step 3: Check Name change on existing account(s)


Step 4: Fill out the table

  • Please ensure you list the card number of the card we are updating the name for. 

Step 5: Sign the bottom of the form



Note: We only update names due to legal name changes. The Credit Support Specialist may require additional documentation in order to process this request.
