How to Close Card - Non-Profit Business

Step 1: Download Business Maintenance Form 

Step 2: Fill out the contact section of the form

• Fill out company name, contact email, phone number, billing address, and billing/control account number
  

Step 3: Choose the appropriate option

• If you are looking to close the full account select Close account(s)
• If you are looking to close only specific cards on other state close cards ending in and list the last 4 digits of the cards you are closing.

Step 4: Sign the bottom of the form
