Update Contact Information - Non-Profit Business

Step 1: Download Business Maintenance Form 

Step 2: Fill out the contact section of the form

• Fill out company name, contact email, phone number, guarantor name and ssn, billing address, and billing/control account number
  

Step 3: On other state update company contact

Step 4: Sign the bottom of the form
